Skilled Nursing & Therapy

Sometimes a single event can change your life. Perhaps it is an unexpected health emergency such as a stroke or heart attack. When difficulties take place, our clinical staff, including our certified Physical, Occupational and Speech Therapists, are available to help you regain independence, mobility and function at your own pace. Our staff respects your needs every step of the way. Skilled therapy can offer a highly effective follow-on regimen to a qualifying hospital stay for conditions including strokes, heart attacks, surgery, broken bones, joint replacement, & respiratory illness.

Long-Term Care

Our long-term care residents receive nursing care while still enjoying daily opportunities to socialize and be active. Our experienced staff supports the daily routine of our residents by assisting with daily living activities and healthcare coordination.

Intergenerational Learning

At The Springs Skilled Nursing & Therapy, we believe in taking care of the whole person — that's why we've partnered with Muskogee Public Schools to create an innovative and interactive intergenerational school, where children and elders talk, play, and learn from each other every day.

Nathan Raith, Administrator

Nathan has been integral to our team for the past 13 years as an Administrator for another one of our facilities. He has consistently demonstrated exceptional leadership, dedication, and expertise during his tenure. Nathan is married to his wife Karen, an RN, and has three grown children — one who served in the United States Marine Corps and another currently in the United States Air Force. Nathan and his wife fill their time outside of work, keeping up with their 8-year-old son, Ethan, who is active in multiple sports. Nathan serves as Secretary/Treasurer on the Board of Directors for the Oklahoma State Board of Examiners for Long-Term-Care Administration (OSBELTCA). He is also an inaugural member and Vice-Chairperson for our Bridges ESOP Employee Communications Committee.
